Coverage for Yourself

You may choose coverage in $10,000 increments up to a maximum of $300,000. You may choose up to $50,000 guaranteed coverage without completing a medical history statement. If you select coverage greater than $50,000 for yourself, you must complete and submit a Statement of Health form to be reviewed by MetLife.

Coverage for Your Dependents

You may choose to purchase coverage for your dependents who are eligible for health benefits with the State in $5,000 increments up to half of your coverage amount (up to a maximum of $150,000). You may choose up to $25,000 guaranteed coverage for eligible dependents without a medical history statement. If you select coverage greater than $25,000 for a dependent, a medical history statement for that dependent must be completed and reviewed by MetLife.

MetLife Funeral Planning Assistance

Planning a funeral is a difficult time for the family and friends of the person who has died. Grieving families may be confronted with dozens of funeral planning decisions quickly and typically under great emotional duress.

As part of the continued commitment to State Employees, MetLife introduces the Funeral Planning Guide as a resource guide to help ease the overwhelming responsibility of funeral planning.

The guide is in an easy-to-use format and provides you the opportunity to communicate your final wishes to your family and highlights details of pertinent information including funeral planning, the death claims process, personal funeral preferences, important insurance and financial information as well as other MetLife benefits that may be available to you or your beneficiaries.
