Important Information Concerning Transition of Medical Care
What is Transition of Care? Transition of Care (TOC) is a program offered by health plans to ensure continuity of care if you or a covered dependent are currently receiving treatment for a serious or unstable condition and you intend to change health plans during Open Enrollment.
What are some examples of a serious or unstable medical condition? If you are receiving treatment for one of the following conditions you may be eligible for TOC: • Last three months of pregnancy • Moderate or high-risk pregnancy • Treatment for cancer • Acute trauma or surgery • End Stage Renal Disease • Awaiting an organ transplant • Treatment for fractures • Bone marrow treatment • Recent heart attack • Joint replacement
When do I need to request Transition of Care? As soon as possible, however it MUST BE prior to July 1, 2009. Please contact the health plan in which you would like to enroll to determine if your current specialist(s) is participating in the plan’s provider network.
