Medication Therapy Management Services

Do you have high cholesterol, high blood pressure, diabetes, or tobacco dependency?  Are you overweight?

Do any of the following apply to you?

• I take multiple medications
• My prescriptions are written by multiple health care providers
• I recently changed medications
• I recently experienced a hospitalization or emergency room visit
• I am having difficulty taking my medication the way I should
If you answered 'yes' to any of the questions above, you should take part in a Medication Therapy Management pilot program happening at State Center. In this program, a pharmacist will work with you and your health care providers to better manage your health. The University of Maryland P3 Program is providing a free annual medication review to ALL employees, and Comprehensive Medication Therapy Management Services (CMTMS) to qualified state employees during the program period from now to June 30, 2014.
What is Medication Therapy Management Services (MTM)?
Meet one-on-one with a clinical pharmacist to review your medications, health conditions, and wellness strategies!

A pharmacist will provide the following services:
• Review all of your medical conditions and medications, including vitamins and dietary supplements
• Work with you and your health care providers to better manage your health
• Provide you with personalized wellness, healthy lifestyle, and disease prevention strategies
• If eligible, you will be referred for Comprehensive Medication Therapy Management Services (CMTMS)
What is Comprehensive Medication Therapy Management Services (CMTMS)?
Review all your medications, including prescription and non-prescription, to determine if they are the best choice for you
• Discuss any drug interactions and if you are having any side effects or undesired effects
• Help you understand what your medications are for and how to best manage them
• Help you understand the importance of routine medical visits, immunizations, and other preventative care measures
• Help you get a better handle on the disease and how to set effective self-care goals
Who can participate: State employees located in 200, 201, 300, and 301 W. Preston Street with active State of Maryland medical and pharmacy insurance
When: Now to June 30, 2014
Where: Lobby level of 201 W. Preston Street, next to Munchie's snack shop
Hours: 9:00 AM to 4:00 PM
